Exit Format - Doc Download

Dear All:
Can you please help on enrty & exit format. If an employee goes out of the organization it has to be communicated to all the other employees saying he/ she wont be there from now on.
Secondly, joining format, if any employee joins our organization we need to communicate that Mr. X has joined has.
Kindly send me the format if you have anything such.
