We Started appraisal process in our company,do any one help me how to post a message to team leads and team members to get their self appraisal ratings,so please help how to bridge the words in a proper way.

Its the time of the year to start the goal setting process for the year 2012-13.
Please fill up Part A-Performance Objectives for the year. The goals set by you will be discussed and mutually agreed by you and your Direct & Matrix Manager. Please send your filled in appraisal form by XXXX.
We can then kick start the appraisal discussions in the week starting XXXXX and close it by XXXX.
