How Do I Ask Team Leads and Members for Their Self-Appraisal Ratings?

Hi, we have started the appraisal process in our company. Can anyone help me with how to post a message to team leads and team members to get their self-appraisal ratings? Please assist in bridging the words in a proper way.

It's the time of the year to start the goal-setting process for the year 2012-13. Please fill up Part A - Performance Objectives for the year. The goals set by you will be discussed and mutually agreed upon by you, your Direct Manager, and Matrix Manager. Please send your filled-in appraisal form by XXXX.

We can then kickstart the appraisal discussions in the week starting XXXXX and close it by XXXX. Please call me in case of any queries.
