How Can I Create a 5-Year Career Plan Questionnaire for My Team? Seeking Advice!

Hi Everybody, I need to prepare a questionnaire that can be used to create a 5-year career plan for individual employees within my organization. Any help on the same will be highly appreciated.

Understanding Employee Profiles for Career Planning

What level are these employees? What's their job profile? What's their qualification? What's their experience? There can't be a standard 5-year career profile for an engineer, a management trainee, an accountant, and a shop floor worker. Unless we know who we are dealing with...??

Thank you for your reply. I don't have to prepare a career plan as such. I need to prepare a set of questions that I can use to ask the concerned employees in order to help them make a career plan.
