How Do I Write a Letter to Request a No Objection Certificate for Loan Foreclosure?

I need help to write a letter to the Manager at Kotak Mahindra, requesting a No Objection Certificate (NOC) for the foreclosure of my loan.

To the Manager,
Kotak Mahindra Bank Ltd.

Subject: Foreclosure of Loan Account No----------------

Respected Sir,

I availed a loan of Rs.-----/- (Rupees in words) against ---------- on dated---/----/------ for the tenure of ----- years in the year------- with a rate of interest ---%. I am regularly repaying the installments as per the agreed terms. At present, I have sufficient funds to pay off the entire loan with interest due thereon. You are requested to kindly let me know the exact amount with interest by the end of this month. Kindly also let me know the foreclosure charges, if any, applicable to me.
