I hope you have PF wothdrawal form with you. If not, you can get it from PF office.
The frist page contains all infroamtion about the employee.
Point no. 3 contains complete name and address of the company
In Point no. 8 an employee will fill the mode by which he/she wants to recive the payment.
Below point no. 8 there will be company's stamp and atuhorized person's sign

left side,
stamp of co. and sign of authorized person
right side, sign of employee
Below it, there is ADVANCE STAMP RECEIPT
paste a revenue stamp on the right hand side where there is a column of signature of memebr and get it signed by the employee.

Employee's bank a/c details. bottom of the page, sign of employee

Advnace stamped reciept same as page no. 2
Bottom of the page
Co. stamp and authorized person's signature.

Leave it blank
7th page
in the place of code no of factory and establishment fill in code somethig like PN/10140 (waht ever your co.'s code), with a co. stamp and authorized person's sign
Fill the form and again stamp and sign
Last page
same as earlier page, stamp and sign below the table and at the bottom of the page.
Do attach PF details which HR dept. hands over.
