Hi Radhika,

I hope you have the PF withdrawal form with you. If not, you can get it from the PF office. The first page contains all information about the employee.

Point no. 3 contains the complete name and address of the company.

In Point no. 8, an employee will fill in the mode by which he/she wants to receive the payment. Below point no. 8, there will be the company's stamp and authorized person's signature.

2nd page

On the left side, the stamp of the company and the signature of the authorized person should be placed. On the right side, the employee should sign. Below that, there is an ADVANCE STAMP RECEIPT. Paste a revenue stamp on the right-hand side where there is a column for the signature of the member and get it signed by the employee.

3rd page

Employee and employer details should be filled in.

4th page

Employee's bank account details. At the bottom of the page, the employee should sign.

5th page

Advance stamped receipt, similar to page no. 2. At the bottom of the page, the company stamp and the authorized person's signature.

6th page

Leave it blank.

7th page

In the place of the code number of the factory and establishment, fill in a code such as PN/10140 (whatever your company's code is), with the company stamp and the authorized person's sign. Fill out the form and stamp and sign it again.

Last page

Similar to the previous page, stamp and sign below the table and at the bottom of the page. Do attach the PF details that the HR department hands over.
