Missed an Employee's Wages in ESIC November '12 Filing: How Can I Correct It?

I have a doubt regarding ESIC monthly return filing. In the month of November '12, in the ESIC monthly filing, I forgot to include one of my employee's wages. We also deposited the challan in SBI. I need to add his contribution to the November '12 monthly returns.

If you forget to mention it in the monthly return, there is no problem. You can add this in November as arrear payments. However, it is wrong if you forget to register him within the prescribed time to send an intimation of new joinings in the nomination form. Please provide his details immediately to the local office or online. Kindly serve a request to this effect that you forgot to register him.

I have registered his name and also got the ESIC number. By mistake, I inputted his 0 days of payments. Now, I want to enter his monthly contribution for November. Can I go for a supplementary contribution? If yes, how can I make the payment to the bank? From where will I get the challan?
