I presume you are talking about EPF. If so, you have to apply in the prescribed form for the sanction of advance/withdrawal. You have to apply in Form 31 prescribed in the EPF Scheme. Details about the scheme are available in this thread - https://www.citehr.com/276387-provident-fund.html
