How Do I Create Effective Training Cards for My Team? Looking for Formats and Tips

Hi friends, I am working as a Senior Officer - P&A with Hindustan Composites Ltd., Paithan, Aurangabad (MS). I have to prepare Training Cards for my employees who have received training and those who will receive training in the future. Can you suggest how to prepare a training card or share a format of a Training Card?

We have designed a training card in which all training records for a particular employee can be kept in a single document. Training needs can be recorded, including training feedback, evaluation, and effectiveness.
