Form For Non Contribution In EPF

Dear Members
Can anybody provide the prescribed form required to be filled in case a employee (having basic+DA more than Rs 6500) doesn\'t want to opt for the EPF deductions

You are required to get filled in Form -XI under PF act which states that employee was not previously memeber of PF scheme. Pls also have backup papers like thier salary slip/undertaking which states that their salary is more than Rs 6500
