EPF Form for Non-Contribution: Compliance and Opt-Out Process

Dear Members,

Could anyone provide the prescribed form required to be filled in the case where an employee (with basic pay + DA exceeding Rs 6500) chooses not to opt for EPF deductions?

You are required to fill out Form - XI under the PF Act, which states that the employee was not previously a member of the PF scheme. Please also provide backup documents such as their salary slip or an undertaking confirming that their salary is more than Rs. 6500.
