In fact in In India the focus on capturing the near misses is very poor. (Exception some companies)
"I always say a mistake (accident) once can be tolerated, but if the same mistake (accident) again, it is a Crime.
EVERY CRIME has a PUNISHMENT.
That is what you meant in responsibility / accountability.

Dear Ramesh,

We are humans and we are born to make mistakes. Few are conscious and the rest are sub-conscious. We do mistakes in both state of mind. Organisations are working with the resolution "ZERO ACCIDENTS" but then how many are successful in preventing accidents. Everything is pre-written and nothing can be changed.

All i can say is that,

- keep educating your employees about SAFETY ISSUES @ WORKPLACE,

- Keep REMINDING them about their FAMILY MEMBERS as they are DEPENDENTS. If anything goes wrong, dependents will be in deep trouble.

- Also motivate your employees to deliver work as per QUALITY STANDARDS without COMPROMISING ON SAFETY ISSUES at workplace.

- Train your employees in such a way, "THEY SHOULD NOT DO ANYTHING WITHOUT FOLLOWING SAFETY RULES STRICTLY' - 'NO SAFETY NO WORK" whatsoever may happen because employees are habitual of compromising with safety rules just to complete their job without analyzing the consequences that may arise.

- Associate ACCOUNTABILITY & RESPONSIBILITY with SAFETY RULES.

- Keep counseling your employees time-to-time as i was doing it regularly to educate our employees on safety rules.

- Last but not the least, SUPERVISOR or the TEAM LEAD must be a ROLE MODEL for their team members when it comes to PRACTICING SAFETY RULES @ WORKPLACE
