How Do I Write a Request Letter to Change an Employee's Name in the PF Account?

I am an employer, and I want to change my employee's name in the PF account. Please help me with the request letter.

Change in Name and PF Account Transfer

Changing the name means transferring the account, which is not permissible under the law. However, you may correct a name that was erroneously written in the PF account due to someone's mistake. This correction must be requested by the member themselves and forwarded by the employer after due verification to the GGN Office. It should be followed up to ensure that while generating the annual accounts, the mistake is not repeated, and the proper correction appears. Once entered, the name becomes the master record with the PF Office.

If the name of the employee in the PF Account is VIKASH SHARMA, but the actual bank account and other details show the name as VIKASH KUMAR, could you please provide me with the format of the letter to address this discrepancy?

With reference, we state that one of our employees, Ms./Mr. Tabassum, bearing PF A/c No. ... & UAN No-..., wants to withdraw his/her PF. However, during the vetting of his/her claim for withdrawal/pension, the following inconsistencies were observed:

The reason for the above variation may be attributable to oversight, clerical mistake, or typographic error on our part. In support of our statement, certified copies of the wage register containing the member’s signature, photo ID proof issued by the employer, or any other document containing the member’s signature are attached.

Besides, any of the following ID proofs (as applicable) of the member are also enclosed in support of our statement: PAN card, Voter Identity card, Passport, Driving Licence, ESIC Card, Aadhar Card, Bank Passbook, Cancelled Cheque, Ration Card, any School/Education-related certificate, Certificate issued by Registrar of Birth & Death, Copy of Electricity/Water/Telephone Bill in the name of the claimant, Letter from a Recognized Public Servant verifying the identity and residence of the member.

We, therefore, request your good office to rectify the member’s name and/or father’s/husband’s name and/or date of birth of the member in your record and process the PF withdrawal/pension request.

We understand that this fact could and should have been brought to the notice of the PF Office during the currency of the membership. However, due to a lack of awareness among the members, it was not brought to our notice, resulting in us not bringing it to the notice of the PF Office in time.

Whatever is stated hereinabove is true to the best of our knowledge. We also understand that making a false statement/representation amounts to a violation of the provisions of the EPF Act and attracts prosecution and other legal cases of fraud.

I/We further indemnify the PF office that if the PF withdrawal/pension benefit is processed in favor of any wrong person due to the above rectification, I/We undertake to remit the entire amount of withdrawal/pension with penal interest and damages as applicable thereon.
