How to Obtain Approval for ITI Apprenticeship Training

Please help me with how to obtain approval for ITI Apprenticeship Training from the government. If any forms are available, please post them.

First and foremost, you can send a written request to the State Apprenticeship Adviser of your respective state for carrying out the survey of your establishment under the Apprentices Act, 1961. Based on that survey, the State Adviser will send a notification specifying the number of seats for apprentices to be engaged in various trades in your organization. Normally, apprentices are to be engaged twice a year in the months of February and October. A Model Contract in triplicate, in the prescribed Format 1 under Rule 14 of the Apprenticeship Act, 1961, should be entered into and signed with each apprentice engaged, and then sent to the Assistant Apprenticeship Adviser for registration.
