if employee not want to deduction their pf which form is fill up what is procedure if if basic salary 7000 i join new company want not deduct my pf can it possible

The Act says that if an employee doesn't have PF in his previous company then he can avail non deduction of PF in case his basic is above Rs.6500.
Form 11 need to be filled for the exemption. Please find the attached Form 11.
