ESI Upload template - reason for zero working days. What are the codes and how do we submit the form?

Hi All, In the ESI upload template there is one column for the reason for zero working days.
What are the codes and how do we submit the form (for April and May) in the following cases?
1. An employee has contribution in April And he resigned on April 30
2. An employee has contribution in April And he is not under coverage of ESI after April 30

Our ESIC registration is at delhi (H.O). We need subcode for kerala branch for 2 employees.
How much will cost for this and what are the procedures?

Instead of Entering in the excel sheet and submitting directly enter in the portal by clicking on the submit button . Just click on the employee name no of days enter wages at the end you will find a preview of the amount to be paid you may confirm from the client and make the payment.
