Need Guidance on Crafting an HR Manual and Employee Handbook: What Should I Include?

Dear all,

I have to create an HR manual and employee handbook for my company and would like to request the seniors to guide me in this regard. I basically want to understand what the basic components of an HR manual and employee handbook are and how detailed they should be. Please help me. If anyone can share a sample HR manual and employee handbook with me, I shall be highly grateful. :)

2. Your Mission, Vision, Quality Policy, etc.
3. Brief about the Company's history.
4. Good HR Practices Policy.

I am now an HR staff member in a medical company and have just started my work for 2 months. I found that there is no HR manual in my company, or it is lost. Could you please send me some samples and let me know the main requirements for an HR manual? 
