I would like to change my surname in PF, superannuation, and gratuity accounts post-marriage. What are the formalities and documents required?

Changing Your Surname in Legal Documents Post-Marriage

Do not change your surname in past documents for at least 5 years after marriage. After that, if you wish to continue your job with your husband's surname, you may first change your ID cards. Then you can apply for a change in legal documents where you submitted them for your benefits.

Firstly, you have to prepare an affidavit (Notary) bearing the changes, then inform your employer of the changes by enclosing an attested copy of the same affidavit. For EPF, fill Form-2 (Revised) with amended details duly attested by the employer and submit the same to EPFO. For gratuity, fill the revised Form-F and submit it to the employer. Lastly, coordinate with your HR Department as they will formalize all the necessary formalities.

In addition to Sumit's suggestion, you have to publish the changes in the gazette and subsequently in the local newspaper too to complete this process.

The gazette copy is the actual authenticated document of the name change effect. It would be better if you preserve the newspaper advertisement along with it.

For a change in surname in PF, etc., you have to apply for a change of surname in the Government Gazette and subsequently apply with a copy of the gazette to the concerned authority.
