Seeking Help: Can You Share Caplan or Cooper's Job Stress Questionnaire for My MBA Project?

I need help from all of you. As part of the fulfillment for my MBA, I am working on a project on "Job Stress." Can anyone provide me with the questionnaire to measure job stress that has been developed by Caplan et al. (1975) - "Job Stress Questionnaire (JSQ)" or Cooper's Job Stress Questionnaire?
