I wish others who want guidance on such formats would post what they have done, rather than asking for 'spoon feeding'.

Comments on the Attachment

Coming to your format, it would have been better if you mentioned the domain/sector your company is in—some sectors have very specific clauses that are emphasized, which may be treated casually in other sectors.

Here are my comments regarding the attachment:

1. Do you intend to conduct medical tests before issuing this Regularization Letter? If not, point (10) doesn't seem to fit.
2. The same goes for point (11)—don't you already have their passport photos in your records? Maybe you can ask for the 'latest' copy.

To issue an appointment letter to employees who have been working for the past 5-8 years but were not provided with a letter of appointment upon joining. Some of these employees have also been promoted or upgraded. Could you please provide a sample format for the appointment letter? Also, what should be the date on the letter that will be issued now?
