My company has 30 two-wheelers which are used by our employees for the official works.
So I need a declaration form to sign by the user for the legal usage of company bike.

Dear Rakesh,
With usage of two wheelers for official purpose is highly risky job. So if your company provides bike then my suggestion is to provide and insist helmets.
