How Do You Document One-on-One Employee Discussions for Future Insights?

I am currently having one-on-one discussions with all my employees, and I would like to document the discussions held so far in a format like Excel. After that, I could generate a presentation on the status, such as satisfaction levels, training needs, feedback about the workplace, feedback about management, and feedback for HR improvement. I am requesting if you have any templates for me to begin with so that I can document as much as possible for the future benefit of the entire organization.

Thank you for sharing the questionnaire; it contains explicit data. As of now, I will be having a one-on-one discussion, which is not related to a question-answer round but rather a discussion on job-related tasks. I need to prepare a report after these discussions and require a PowerPoint format to present to management the statistics of my HR discussions with employees and suggestions for improvement.
