There are many forms in PF and ESI.
here i am giving a brief description of PF forms for your info.
form 2- employee decleration and nomination form.
form 5- Return of Empoyees qualifying for membership of the Employees' Provident Fund ,Employees Pension Fund & Employees' Deposit Linked Insurance Fund for the first time
form10-Return of the Members Leaving Service During the Month
form 10 C-To claim with drawl benefit
form 13- Transfer of Pf account

Form 10C- EPS Settlement form for Pension Amount.
Form 13- EPF & EPS Account(Amount) Transfer From One company to Another Company.Transfer Form.
Form - 5 List of Addition
Form -10 List of Deletion.
Form -31- Partial Withdrawal Form(loan Form)
Form - 10D- EPS From Getting Pension for After Retirement.
Form - 2 Nomination Form.
