Seeking Advice on Creating a Candidate Evaluation Form for HR and Technical Reviews

I am working as an Executive HR in a power projects company. I am preparing an evaluation form for the candidates. This evaluation form will help in evaluating the candidate by the HR and also by the Technical Person. Friends, please provide your inputs on preparing this form with some samples.

The evaluation forms need to be customized for your requirements. The behavioral parameters and technical parameters need to be separate. The parameters should have weightage in the evaluation. The evaluation forms are too generic to be directly useful to you.
