Information Required While Taking ESIC Code For The First Time For The New Company.

Hello Everyone
I have just opened a small company and i want to know what are the documents required for taking a ESIC registration.we have more than 200 employee including all the grades.
we are basically a facility providing company.

The following documents need to be submitted while taking new ESIC License,
1. ESIC Registration Form
2. Labour License
3. Company/ Partnership Incorporation Certificate
4. MOA of Company/ Partnership
5. Address proof of the company (Electricity Bill/ Telephone Bill)
6. Bank Statement
7. Make a DD of 6.5% of contribution from ESIC Covered employees salary. Calculate one months contribution multiply with 6 and make a DD and pay the same to the department as a security deposit.
