Is There a Manpower Request Form to Streamline Monthly Recruitment Needs?

I am into recruitment. The center heads of the respective production centers will give me their manpower requirements for different positions on a monthly basis. Their requirements will be either mailed or verbally communicated. I want to know if there is a form called "Manpower Request Form" that can be circulated to the center heads at the beginning of each month. They can then fill in their requirements on that request form and mail it to me.

If anyone can provide such a form, it would greatly help me.
