HR Procedures To Grand Maternity Leave For An Employee

Kindly advise me on what all HR procedures are to be followed inorder to grand maternity leave for an employee. Like, Is there a Maternity Leave Application Form to be filled by the employee? Should the ID Card & Swipe Card be taken back from them during the leave period? Please reply on what all procedures are to be followed..

-> Yes, they need to fill a leave application from.
Should the ID Card & Swipe Card be taken back from them during the leave period?
-> Depends on your organisational culture. Generally when a person takes a leave (for any purpose) they need not to submit the id card or swipe card.
A female employee can avail Maternity benefit under two categories:
A) Under ESI
B) As per Maternity Benefit Act if not covered under ESI.
Please refer the two acts.

Female Employee have to fill this form before 1 month of availing Maternity Leave with certificate of registered medical practitioner's stating expected delivery date.
