Former Employee's PF Withdrawal Request: How to Handle Name Mismatch and Ensure Success?

One employee left the company a year ago and today came asking for a PF withdrawal amount. I processed Form 19 and 10C. If it is sent, will it be processed successfully? Additionally, his name in the bank account does not match our records.

Yes, it will be processed, but he has to enclose the notary affidavit in the original to confirm his name.

Hi Shanti, no issues. You should submit Form 10 and 19. There is no issue with the bank account number. The only thing is you should enclose the canceled cheque of that employee along with Form 10C and 19. It is better to enclose copies of Form 5 and 10 and Form 3A for the relevant periods.

You can get a letter from the employee stating that, at the time of joining, they provided their nickname instead of their original name as per government records. Additionally, you should attach the school records or a ration card Xerox duly attested by you. A notary affidavit is not required. This letter, with enclosures, can be attached to Form 19 & 10C, duly attested by you.

Thank you. Please submit Form 3A for all the years along with the resubmission of Form 19 & 10C for the specific employee. This will enable the PF office to expedite the process for early settlement.
