Dear Dilip, Thank you for sharing the incident study. However, upon analyzing the incident, it was evident that the welder did not take proper safety precautions for the welding work. Measures such as ELCB/RCCB, proper hand gloves, safety shoes, etc., were not utilized.

This type of incident control is done by using a work permit system. The work permit ensures that contractors use ELCB (30 mA Capacity), proper and adequate personal protective equipment, LOTO, etc.
