Effective Performance Appraisal Format for Skilled and Unskilled Workers in the Construction Industr

Kindly provide me with a good performance appraisal format to rate both skilled and unskilled workers in the construction industry. I would like to evaluate the workforce using a direct rating system. The input of the evaluation criteria is crucial in formulating the format.
