New HR Executive Seeking Guidance on Filling Out Adult Workers Register – Any Tips?

Hello, friends. I am a fresher and have just joined a company as an HR Executive. Can anyone help me maintain or fill out the register of adult workers? There are columns like:

1. Letter of the group in Form No. 11(b).
2. Number of the relay, if working in shifts.
3. Number of certificate and date if an adolescent; number of certificate and date, token number given as reference to the certificate.

I will be so thankful if someone can help me with this.

Can anyone help me with how to fill the register of adult worker column no. 6 (letter of group as in Form 11(b)) and column 7 (number of relay if working in shift)?
