Struggling with Form-J, Rule 20(1) Register for Night Shifts: Can Anyone Help?

Understanding Form-J, Rule 20(1) Register

I have a register of Form-J, Rule 20(1) (we work on a night shift basis; it starts at 9 pm and ends at 5 am in the morning) for the Register of Employment in Commercial Establishments. However, I don't understand how to fill it. It is different from the attendance register we have. Can anybody please explain how to maintain this register? Please reply soon.
