Why you are targeting Safety Engineer????? !!!!!
The then supervisor of the worksite is 100% accountable for the accident. Why because he is supposed to make sure that everything is in place prior to the commencement of work.
Try to identify the real cause rather than put blame on others!
Safety people can only highlight the situation but the actual implementation should be from the process owner side.

Is there any person / team who is responsible for regular maintenance and to ensure the workplace safety?
Please do investigate .In that case ,the responsible person should be liable and a letter of warning can be issued to him .
In the cases where maintenance team maintaining the checklist should be liable for .
Also need to identify at the time of accident the wounded person know its use properly or not or having knowledge on basic precaution or not.

This letter is to inform you that a serious complaint has raised against you due to recent accident that occurred scaffold board broken where lack of proper fall protection and safety and caution are reasoned for.
