Requirement of ID Proof and Bank Statement for PF Withdrawal

Is ID proof and a bank statement copy (in which the PF amount will be settled) required along with Form 19 for the withdrawal of the PF amount? Some employers are requesting the submission of ID proof such as a driving license or PAN card, along with the last month's bank statement.

For the withdrawal of P.F., you require the following documents:
1) One Cancelled Cheque (The Bank should have IFSC Code)
2) Address proof (Electricity, Telephone Bill, etc.)
3) If you are going to withdraw the PF after 3 years (after leaving the job), then an Indemnity Bond on Stamp Paper of Rs. 100/- with a notary.
