Hi Ankita
If your colleague has allotted the same number for 2 different employees then one of them will be a wrong account number w.r.t that employee where the contribution would become nil to him and i think you need to make the fresh contribution with the penalty charges as below
less than 2 months 17%
2 months but less than 4 months 22%
4 months but less than 6 months 27%
6 months and above 37%
you have the provision to recover the double contribution made to the same account number under sec.32(1) which says Recovery of a members share of contribution :-
Provided further that where no such deduction has been made on account of an accidental mistake or a clerical error, such deduction may, with the consent in writing of the Inspector, be made from the (Subsequent) wages.

Hi Ms.Ankita,
NCP means Non contribution period.if any NCP days are for the members mention the days for the respective members, otherwise mention 0(ZERO).
About same a/c number for two members submit the revised Form No.5 for Dec 2011 with covering letter, so problem will get solve, then file annual returns.
Thanks

HI Ankitha ,
You write a covering letter to Asst commissioner of PF , ........ Branch, stating that due to clerical mistake the same A/c No...was alloted to two of the employees and request him to change the same by attesting the Ammended Form-no-5 and 6A . Make corrections in form-9 of PF Office.
