Struggling with My MBA Project on Customer Attrition in Cellular Services – Can You Help?

I am a final year MBA student. As part of our curriculum, the summer project has started, and I am facing difficulty in framing a questionnaire. My topic is "customer attrition towards cellular services," and I need help from everyone. Please feel free to assist me in this matter.

Have you studied Research Methodology as a topic? If not, please see my post on that. The better way to seek help is to design a questionnaire clearly stating the aims and objectives of your survey, how many responses you expect, and what hypotheses you want to prove, etc.
