Seeking Performance Appraisal Formats for a Construction Company: Any Suggestions?

I am working as an HR & Admin Manager with a construction company based in Mumbai and our regional office in Bangalore. I am responsible for the HR & Admin functions at the regional office where the total manpower strength is 50 employees. I need to create performance appraisal formats for our company.

We operate in the construction and real estate development industry. Could you please suggest performance appraisal formats suitable for this sector?

Thank you for the formats. However, these formats are not providing a clear picture of the candidates' functional competencies as well as their managerial competencies. I have a format that was used in my previous company. I have enclosed this format for your reference. I would appreciate some feedback on whether this format is suitable for the construction industry.
