Laptop Security Form - Doc Download

we give laptop's to all our employees upon joining the firm. I need to take an acknowledgment regarding the laptop secuirty and also the terms and conditions apply if they loose it. Please send me a form which has all these details mentioned in it so that i can get it signed by all my employees
