Struggling with Form No. 10C and 19 Mistakes? Need Sample Filled Forms for Guidance

Dear seniors, I have sent my Form No. 10C and 19 to my previous company. After 6 months, they returned it with a letter stating that there are some mistakes in my filled forms. Can anybody upload a sample of filled Form No. 10C and 19?

First, ask for the exact reason for rejection and what those mistakes are. I believe you might have missed some details. Moreover, you are only required to fill in your personal details; the remaining details should be filled in by your employer. 
