How Do I Create Effective Evaluation Forms for a 90-Day Trainee Program?

Can you help me with the evaluation form for the trainees who will later become employees? I need an evaluation form for a 90-day training program where each month has a different evaluation form.

Please assign this job to the trainer who conducts the training program, whether an in-house trainer or engaged from outside. Training evaluation after the training will only reveal the 'knowledge' obtained or updated during the training. It is essential to evaluate their performance and then relate it to the training efficacy. Hopefully, a pre-training evaluation has also been conducted.

What do you mean by pre-training evaluation? I'm the training officer, and I need to do this form. 
