Thank you, Raghu, for posting. As is my habit, I checked to see if this was copyrighted text. I found links to over 600 fatality reports under the caption "Workplace Safety & Health Topics" by the Centers for Disease Control and Prevention at CDC - NIOSH Fatality Assessment and Control Evaluation (FACE) Program: Investigation Reports - NIOSH Workplace Safety and Health Topic. You can access the reports at CDC - NIOSH Fatality Assessment and Control Evaluation (FACE) Program: Investigation Reports - NIOSH Workplace Safety and Health Topic.

There is no need. I have given a link where you will find that report among other things. The detailed report, including recommendations, was published in 1990 and is at FACE 9011.

When we receive something by email, it is easy to check the source by Googling. Then we can find out whether it has been published elsewhere or whether it's a hoax.
