NDA I.e NON DISCLOSURE FORM For A BPO

I want an NDA i.e NON DISCLOSURE FORM for a BPO.
There is a different type of NDA NOn disclosure form for a BPO company . I need a sample format for the same.
I will be greatful if anybody can help me for the same

Hello All,
I want and undertaking form that includes the statement that if employee is currently working with some company he/she can not go for employment in any other company at that time.
I have found undertaking for various scenarios but not for this same thing that I am looking at.
Please help me out for the same.
