TDS Declaration Form For FY 2012-13 With New Amedtments

Dear Sr.
Please provide me TDS Declaration form for the FY 2012-13. I tried on google but cant find.
I need it with the recently made changes.

Good morning :)
Actually I need TDS declaration form which we get filled from employees. In this form employees fill amount of their investment plan in different section for the following year.
Mediclaim,
PPF
Infrastructure
LIC premium
Children tution fee
Loan
Interest on loan etc.
I have form form for year 2011-12 but my colleagues said that that is wrong because from this year few sections have changed which I dont know.
Changes are made in Sec 80 C , point 10 , infrastructure bond.
You can see the old one attached herewith.
Please help me ,

Hi Jeevarathnam ,
I think Infrastructure Bond is now named as " Rajiv Gandhi Equity Saving Scheme" and amount is increased from 20000 to 50000 yrl.
This is the only change rest all slabs would remain same.

I have worked with a company for 5 months,then I resigned but my resignation has not been accepted,however another company was ready to appoint me without resignation acceptance.I have joined the other company.Now 2 years so far has passed.My question is this weather i can take my PF from previous company.

You could have requested for PF trnasfer. Even now you can request for transfer. Approach your HR He/She shall do the needful.
