Activity Planner For The Month

Hi,
I am looking for an monthly Activity Planner Format. Can Anybody Please help me and if possible kindly tell me what should we fill in it as Plan for the coming month as it is very difficult to plan HR activity in Advance. Seniors please help.

Thank you Peer, for sharing such a wonderfull planner however I am looking for much simpler planner in which we can ask employees to plan thier work for coming month and share it with thier reporting manager and at the end of month we can evaluate how much was achived and what was the reason of deviation if any.
