How Can We Create an Effective Training Questionnaire for OJT Students in Biomedical?

Hi HR Partners! I am new to this site/group and seeking your professional expertise to help me create a training questionnaire for on-the-job trainees (students) to evaluate the training program within the company.

Objective of the OJT Program
