How Can I Correct Name Errors on P.F. Slips for Factory Workers? Seeking Guidance

This is Varma, working as an HR in a cement manufacturing company. I have received many complaints from the workers of our factory regarding the spelling mistake of their names on the P.F. slip. Can anyone kindly help me out by telling me the procedure for applying to the P.F. office for the correction of their names? Please help me out by letting me know if there is any specific form for that.

When you receive the PF slips every year in October, check the names of your employees and send them back to the EPF office. The corrections will be made and regenerated in the next PF slips.

Procedure for Name Correction on PF Slips

On your letterhead, write to the EPF officer, stating the old name (Wrong Name) and mention the correct name within six months of receiving your PF slips. The letter should be signed by an authorized signatory, i.e., a person with the authority to sign on PF monthly returns and withdrawals.

Mr. Sunil is correct. I have done the same thing in my company. Also, enclose Form No. 5 and the employee's ID proof photocopy. Make an application on the company letterhead (two copies): one you have to submit there, and another received copy you can file and keep in your records.
