How Do I Create a Monthly PPE Sheet to Track Usage by Department?

Could someone provide me with a format for a PPE Sheet? I would like to create a Monthly PPE Sheet to track the amount of items used by each employee in every department.

Thank you for the attached sheet; it is useful. However, I would like a sheet that includes the employee names and lists each equipment used by every employee in the month. This sheet should be kept by the supervisors and filled out by them.

Creating a PPE Sheet

It's very easy to create it in an Excel sheet. You just need to include the following columns: Name, Equipment Name, Designation, Required PPE, Issued On, Supervisor Signature, and Employee Signature. That's it, you can make it. Let's try, bro.
