How Do I Write a Confirmation Letter Request After My Probation Period?

I am working in a company (the company is into building materials) that employed me as a sales clerk. I was given a three-month probationary period, which I completed in January 2012. Now, I have discussed it with my direct boss verbally, and he said I should put it in writing and apply for a confirmation letter. How can I write it? Please guide me.

Good day, I have been working with a company for six months now, and I was given a three-month probation period which I completed two months ago. I was waiting for my confirmation letter after the probation period ended, but I did not receive any information about it. Consequently, I approached my direct boss to address the issue. He asked me if I had applied for the confirmation letter, to which I responded negatively. I also mentioned to him that my salary is lower than that of every other staff member at the same level, with even junior staff members earning more than I do. My boss advised me to apply for the confirmation letter and to include the salary concern in my request letter.
