I think, the PAF should have certain sections, the frist part, which is done by the employee should include the personal data with the job description, the date from and till when this particular report is covered. the Second part should include the employers remark as mentioned in Amits form, and the third should include a common part, which mentions the consent of the employee stating that the PAF is discussed with him and is agreed, the approval of the supervisor and the signature of the section head.
If your organisation has a HR department, then the signature should be made in front of the HR personnel as the witness.

i have to prepare an appraisal form for Recruiters,,,so can anyone help me out in that...if possible kindly help me with some more formats of the appraisal forms so that it can help me to make a perfect form....

I do not know what is the level of your company's HR management. are you talking of the very early stage of developement or with matured system of appraisal of performance by objective setting, 360 degree, internal customer feedback, etc.
however, from your mail, you seems to be a beginner. pl. find attached form, which may be useful to you in your later career.
according to your company's activities, modify the contents.
