Confused About Form-D in the Textile Industry? Need Help with Format and Frequency?

I am working in the textile industry. An auditor asked me to show the Form-D (Equal Remuneration). However, I am not aware of it until now. Can anyone tell me the format and how to fill it out? Is it filled out monthly or yearly?

First, you can categorize the workmen and mention their remuneration (Minimum Wages) for both men and women without any discrimination.
