I was also looking out for the same. Our appraisal is in June, and it is an industrial sales and service company. I have created a performance sheet that includes employee details, experience, qualifications, professional knowledge, and personal knowledge. Additionally, there is another sheet that includes their goals with their targets and achievements, as it is sales and service-related.
Last time, we had set some goals for each employee, so I will include all those details and hand them over to the concerned person. They can gauge and write the feedback themselves, and then that sheet will be passed on to their Head of Department. This way, at least some information will be provided for the appraisal.
