Performance Appraisal Of Worker, Apprentice, Supervisor

Hi All,
We have decided to conduct performance appraisal of workers, Apprentice / trainee & Supervisors. This will be very difference from staff. It will be more focus of achievement of production target / milestones. We are in process of collecting inputs from varios departments and seniors.
If anyone of you have any input or format / template, please share with me.
