Seeking Input on Performance Appraisal Formats for Workers and Supervisors: How Do You Measure Success?

We have decided to conduct performance appraisals of workers, apprentices/trainees, and supervisors. This will be very different from the staff appraisal. It will focus more on the achievement of production targets/milestones. We are in the process of collecting inputs from various departments and seniors. If any of you have any input or a format/template, please share it with me.
